I, (Guardian Full Name*) am the legal guardian of (Minor Full Name*), and hereby authorize the 49th Waypoint to utilize photographic and videographic image(s) of the Minor in its public social media posts, communication materials (published and/or printed), which may include (but are not limited to): public internet pages, social media advertisement, etc. In giving my consent, I hereby release and hold harmless the 49th Waypoint’s executives, members, and representatives from any and all responsibility or liability for any kind of damage suffered in any manner whatsoever related to the use of the photographic image(s) and video(s) of the Minor. I hereby relinquish any and all personal or proprietary rights of which I may have in connection with such use of the photographic image(s) and video(s) of the Minor. I understand that I will in no way receive any compensation should any photograph or video of the Minor be used or published. I understand I may revoke this consent at any time by providing written notice to 49th Waypoint. Such revocation will not affect materials already produced or published. This collection, use, and disclosure of personal information is in accordance with the British Columbia Personal Information Protection Act (PIPA).
